New Dental Patient Registration

During their first dental appointment, new patients are required to provide our practice with details such as name and address, medical history, payment responsibility, etc. This information is necessary for staff to maintain accurate, up-to-date dental records.

Your time is valuable and you can reduce the time spent filling in the forms at our office simply by downloading and printing them; then fill them in at home and bring the forms with you on the day of your appointment. You may also Fax them to: (770) 234-5458.

A Convenient Option
You'll need Adobe installed to access the PDF documents. While we suggest you print and fill in our registration forms before your appointment date, it's just an optional convenience. You can of course complete the forms here at our office if you like -- on the day you come in.
